Job Description:
A company is looking for a Licensed Clinical Social Worker
(LCSW) to provide behavioral health services as part of an
interdisciplinary care team.
Key Responsibilities
Conduct remote patient assessments and provide solutions-focused
interventions
Develop and adjust behavioral health care plans based on patient
progress
Collaborate with providers and community health partners for
coordinated patient care
Required Qualifications
Bachelor's or Master's Degree in social work
3 years of clinical social work experience within an
interdisciplinary care team
Valid and current LCSW license
Value Based Care experience preferred
Comfortable with digital technology and various EMRs
