Case Manager either of 2 Locations:
Case Manager with Behavioral Health experience for the SMI Team
(NOVA area).
Case Manager with LTSS experience (Lynchburg, VA).
Responsibilities include:
Developing care plans, participating in interdisciplinary meetings,
and maintaining accurate records.
Strong computer skills and attention to detail are essential.
This is a fast-paced role requiring multitasking.
Travel: 30%+ local travel for member visits (mileage
reimbursed).
