Job Description:
A company is looking for a Full Time Pediatric/ Neonatal
Intensive Care Unit Complex Case Manager Registered Nurse -
Remote.
Key Responsibilities
Collaborate with members and interdisciplinary care teams to
develop personalized care plans
Engage with members via telephonic communication to provide
education on services and clinical program enrollment
Conduct behavioral and clinical assessments to identify holistic
needs and advocate for appropriate care
Required Qualifications
Bachelor's degree in nursing (BSN) or equivalent
Compact licensure
Current/active Certified Case Manager License (CCM) or commitment
to obtain within one year of hire
Minimum of 5 years as an active Registered Nurse with a current
unrestricted license
At least 3 years of experience as a Neonatal Intensive Care Unit
(NICU) Registered Nurse
